• Please mention, when booking your appointment, that you are attending for a Driving Licence Eyesight Test or a Driving Licence Medical (Group 1 or 2) 

• The relevant forms are available here for downloading

 

Please complete as much of the form as possible before your appointment. 

NB: The Form MUST be printed out on 2 sides (1 sheet of A4 paper) and completed in BLACK biro.

Insurance Medicals & Occupational Health

• We provide an Occupational Health service for a number of local businesses and firms. The service includes carrying out pre-employment medicals, assessing employees who have injuries in the workplace, giving vaccinations
