CHRONIC DISEASE MANAGEMENT PROGRAMME

• HSE initiative, phased in during 2020

• Initially targeting over 75 year olds , other age groups to be phased in 2020 - 2022

• Eligible Patients will see GP’s/Nurses several times a year in a dedicated clinic

• Eligible Conditions: Heart Failure, Ischaemic Heart Disease, A Fib, previous stroke, Asthma, COPD and Diabetes.
